Rita Formby, Administrator
I have been in Long Term Care for fourteen years, beginning as Assistant Director of Nurses for National Heritage/Pierremont, and quickly promoted to Director of Nurses and transferred to Westwood Manor. While there my Administrator encouraged me to become an Administrator myself.
I became employed with Central Management Company in 1989 as Administrator-In-Training at Magnolia Manor. After completing my training I was appointed Administrator and remained at that facility until transferring to Garden Park in July 1997.
I have served as President of Nursing in Action for Region VI, and have served on various committees with long term care such as Senior Olympics and the Alzheimer's Walk.
I was selected as Administrator of the Year for Central Management Company in 1992 and again in 1998. I was also nominated for Administrator of the Year for Louisiana by our region.
Mary Bordelon, Director of Nurses

Director of Nurses at Garden Park, from 1997-present. Served as Director of Nurses at Heritage Manor in Bossier City, Louisiana, from 1995-1997, and as Assistant Director of Nurses there as well, from 1994-1995. At Centenary Heritage Manor served as Director of Nurses from 1984-1994. Also served as Charge Nurse at Willis-Knighton Hospital in Shreveport, Louisiana (1977-1981), as Charge Nurse at Shumpert Medical Center in Shreveport, (1974-1977), as Registered Nurse at Children's Hospital in Birmingham, Alabama (1972-1974) and as Registered Nurse at Max A. Curry, M.D. in Biloxi, Mississippi. Received Associate Degree in Nursing from Jefferson Davis Jr. College in 1969.
